You are voluntarily consenting to receive physical therapy services rendered by Hilary Garrett, as ordered by your physician or nurse practitioner and physical therapist. Please understand that you will be informed of all proposed medical procedures or treatment before receiving them. You have the right to refuse any proposed medical procedure or treatment. You are also authorizing Alpine Physical Therapy to release medical records to your physician and/or insurance company.

CODE OF ETHICS

Hilary Garrett, MS PT and Diana Spring, DPT, FAAOMPT endeavor to thoroughly comply with all applicable federal and state laws, including laws that prohibit the filing of false or fraudulent claims. In consideration therof, this practice shall bill only for service rendered, document medical necessity and appropriateness, code accurately, and adhere to all payer contracts. This commitment is fundamental to the organization and all employees and contractors are expected to support our commitment. Employees will be evaluated in part on their contribution to ensuring that our organization meets this commitment.
