*When you sign the registration form, please read the Assignment of Insurance paragraph carefully. You are authorizing direct payment of medical benefits to Alpine Physical Therapy. However, you are acknowledging that you are financially responsible to APT for all therapy services regardless of how much is covered by insurance or other third party payers. Be aware of our cancellation policy and our billing policies.
