*When you sign the registration form, please read the Assignment of Insurance paragraph carefully. You are authorizing direct payment of medical benefits to Hilary Garrett. However, you are acknowledging that you are financially responsible to Hilary Garrett for all therapy services regardless of how much is covered by insurance or other third party payers. Be aware of our cancellation policy. Also, be informed that if your account is over 60 days receivable, you will be assessed a re-billing fee of $10 per month for every month your account remains unpaid. Additional fees will be assessed if your account is turned over to a collection agency.
