Cancellation Policy  

To better serve our patients, we enact this policy:

• You are responsible to be present during your scheduled time for physical therapy treatment. If you are more than 15 minutes late, you will have to re-schedule your appointment, and face penalties for a missed appointment.

• You are required to call to cancel your appointment with 24 hours notice if you are unable to attend.

• You (not your insurance company) will be charged a fee of $25 for cancellations with less than 24 hours notice or missed appointments without notifying your physical therapist.

• Your referring physician will be notified if you have more than 2 missed appointments without notice or with insufficient notice.

PLEASE BE CONSIDERATE OF OTHER PATIENTS WHO MAY BE WAITING FOR AN APPOINTMENT BY CANCELLATION WITH SUFFICIENT NOTICE.
